The HEart Failure Discharge Initiative (HEFDI) group was formed to develop hands-on solutions to overlooked areas which can positively impact the continuum of care for heart failure patients and potentially reduce readmissions. The group is comprised of clinicians with decades of expertise in both the care and research of heart failure patients.
This webinar discusses evidence for the utility of natriuretic peptide testing to improve outcome, and presents case studies on the management of heart failure after admission and discharge.
