About the HEart Failure Discharge Initiative


The HEart Failure Discharge Initiative (HEFDI) group was formed to develop hands-on solutions to overlooked areas which can positively impact the continuum of care for heart failure patients and potentially reduce readmissions. The group is comprised of clinicians with decades of expertise in both the care and research of heart failure patients.

This webinar focuses on factors associated with early rehospitalization of patients with heart failure, specific interventions which reduce readmissions, and strategies to enhance post-discharge outcomes. Participants are also introduced to HEFDI resources.
